Immunization clinics planned in November

Free immunizations are available for children and adults at various sites in Chippewa County each month.
Immunizations will not be given for any children under age 18 years unless a parent or legal guardian fills out and signs a permission form.
Immunization consent forms can be printed off the Chippewa County Department of Public Health Web site at www.co.chippewa.wi.us. Bring and present the child’s immunization history.

-- 9:30 to 10:30 a.m., Tuesday, Nov. 4, Cornell Elementary school lunchroom, 417 Main St., Cornell. By appointment only.
-- 9 to 10:30 a.m., Thursday, Nov. 13, Bloomer Baptist Church, 2620 Duncan Road, Bloomer.
-- 1:30 to 2:30 p.m., Monday, Nov. 17, Our Savior’s Lutheran Church, 147 E 4th Ave., Stanley.
-- 8:30 to 10 a.m., Tuesday, Nov. 25, Public Health Department, Room 123, on the first floor of the Chippewa County Courthouse, 711 N. Bridge St., Chippewa Falls. By appointment only.
